Please print as clearly as possible all information on the form. You will receive an E-mail back to confirm the receipt of your fax within 48hours. Please allow 3-7 days to receive your gift card by U.S. postal mail. Chow cannot be responsible for gift cards lost in the mail or lost or stolen after receipt.
